Heiken Children’s Vision Program

The Florida Heiken Children's Vision Program provides comprehensive eye exams and glasses for Florida public school students who qualify. This program is available statewide at no cost to you or your child’s school and gives financially disadvantaged students who fail their vision screening the best chance at academic success. 

The comprehensive eye exam is administered by a licensed optometrist and includes a thorough examination of your child’s vision and eye health. If your child is eligible to participate after verification, your school will provide a voucher with the name, address, and phone number of a participating doctor. To receive free services, the voucher must be presented to the assigned doctor at the scheduled appointment.

Families can apply now on the Heiken Portal. All student information is kept confidential and not shared with any other entity.

If you are unable to apply online, download and complete the consent form. Consent forms can be submitted to the nurse in your school clinic. Please note that consent forms will take longer to process than online applications.
